Medication error causes stroke

We were called today by a veteran who had been taken off of his high blood pressure medication by the VA which directly led to a stroke causing him eyesight loss and brain damage. He is able to work, but is not allowed to drive. His stroke was totally preventable. When he was on the blood pressure medication he improved so he was taken off but his pressure went back up and he repeatedly asked for his medicine to be prescribed again since home blood pressure checks showed his pressure to again be elevated. He was refused and then came the stroke from his high blood pressure. This stroke was preventable. Medication errors often lead to injury. We will be filing a claim against the VA under the Federal Tort Claims act for him.
